Careplan\u2019s Care Managers<\/strong><\/a> will advocate for your family at medical appointments ensuring the physician has all the facts.<\/h4>\n<\/div><\/div><\/div><\/div><\/div><\/div>

Our Care Managers facilitate communication among care providers, doctors, and families. They also monitor seniors\u2019 adherence to medications, medical orders and instructions.<\/p>\n

Our Care Managers will:<\/strong><\/p>\n<\/div><\/div>

  • Coordinate and arrange for transportation<\/li>\n
  • Attend medical appointment and report back to family<\/li>\n
  • Advocate on the patient and family\u2019s behalf<\/li>\n
  • Assist with Pre- and Post-Surgery needs<\/li>\n<\/ul>\n<\/div><\/div><\/div><\/div><\/div>

    • Oversee arrangement of follow up appointments with specialists<\/li>\n
    • Monitor and see that physician\u2019s recommendations are being followed<\/li>\n
    • Provide home visits to monitor for wellness and change.<\/li>\n<\/ul>\n<\/div><\/div><\/div><\/div><\/div><\/div>
