Bridget Ritossa, LSW, CMC – Aging Life Care Manager and Founder
Careplan Care Managers was founded by Bridget Ritossa, Certified Care Manager, Licensed Social Worker. Bridget developed a wealth of knowledge asresources in the areas of aging. This is a result of her 20 years of practice working at two major hospital systems in Cleveland. As a matter of fact, she worked at two of the largest healthcare chains in the country.
Her grandmother lived with her while growing up. As a result, this helped develop a genuine love and appreciation for the care coordination.
Studies have shown that living at home is better for the mental health of individuals. This is because they are happier in a familiar environment where they can live independently. However, when home is no longer the best option, Bridget will help families to find the most appropriate placement with her extensive experience in residential care settings.
Above all, Bridget’s goal was to create a company that would provide adults with the resources and knowledge to live well. Therefore, creating an individualized care plan is at the very heart of Careplan Care Managers.

Stacie Hackleman, LSW, ACM-LSW – Aging Life Care Manager

Stacie serves the Eastern as well as Western Cleveland area. As a Licensed Social Worker and a long-time Cleveland resident, she has extensive experience supporting people in the community. She also has a background in rehabilitation and case management services. In addition, she works PRN for The Cleveland Clinic Foundation. Moreover, Stacie has assisted hundreds of people transition from hospital to home. In addition, she has significant experience with rehabilitation services, entitlements, Medicaid and Medicare.
Stacie lives in a suburb of Cleveland with her husband, two sons, and dog. She enjoys traveling, being outside, working out as well as spending time with her family.
